The OnCare Transition Age Youth Project is a coordinated system of behavioral health services and community supports for young adults, ages 16-25, with mental health diagnosis, substance abuse concerns, juvenile and young adult academic/employment and/or social challenges. Case management and referral services are delivered via the Credible Messenger approach by the OnCare TAY staff that consist of a Transition Facilitator Supervisor, Peer Young Adult Coordinator, Training and Technical Assistance Coordinator, and Transition Facilitators, many of which have “lived experience” and utilize the Transition to Independence Model (TIP).
