NYSDOH Drinking Water Operator Certification - Application Process:
1. Complete the Application for Certification (Water Treatment Plant or Distribution System Operator - DOH-354 (PDF) form and include the following:
- a letter of verification prepared by a water supply official (supervisor) outlining the appropriate water operator certification grade level duties and endorsing your application for certification
- the initial NYSDOH-approved grade level course certificate(s) of completion
- a copy of high school and/or college diploma(s)/college transcript(s).
2. Email or mail the required application documents to the Onondaga County Health Department. Please note, however, that the required application documents can also be turned in during the On-Site Assessment.
3. Contact the Onondaga County Health Department Public Water Supply Section at 315-435-6600 to schedule an On-Site Assessment.

General Information
1. Create a NY.gov Account.
2. Log into the NYSDOH eLicensing Portal to fill out and submit an application. (Please use one of the following web browsers: Internet Explorer or Firefox.)
