The Onondaga County Health Department is currently accepting applications for the position of:

Public Health Social Worker I

Location: 421 Montgomery Street, Syracuse, NY

Salary: $57,738

Hours of work: Monday – Friday, 8:30 a.m. – 4:30 p.m.

The Onondaga County Health Department’s Bureau of Disease Control (BDC) offers services to the community focused on the management of Tuberculosis, Sexually Transmitted Infections, and Communicable Disease. The BDC currently has a full-time opening for a Public Health Social Worker I (PHSW).

This position is 100% grant funded, and focuses on supporting patients in the STD Center. The PHSW will meet with patients to conduct a brief social work assessment and provide short term case management services and crisis counseling, with an emphasis on connecting patients with mental health supports and other community resources as needed.

A portion of the PHSW’s work will also include responsibilities related to grant management, which includes but is not limited to: gathering data and writing narratives for monthly reports and/or new grant proposals, conducting quality improvement projects, submitting purchase requests for supplies, attending and presenting at required meetings, writing/ updating social work policies, organizing community outreach and awareness events, collaborating with community partners, and providing training for Outreach Workers.

Candidates must be licensed as a Master Social Worker by the NYS Department of Education. Experience working in the field of sexual health is preferred.
