The Onondaga County Health Department Lead Poisoning Prevention Program is currently accepting applications for the position of:
Outreach Worker (Health) – Full-Time Permanent
Location: 421 Montgomery Street, Syracuse, NY 13202
Salary: $33,990 per year
Hours of work: 8:30 a.m. – 4:30 p.m., Monday – Friday - Occasional night and weekends hours
The Onondaga County Health Department Lead Poisoning Prevention Program (LPPP) is seeking a qualified candidate for an Outreach Worker (Health) position. The Lead Poisoning Prevention Program monitors all blood lead testing results of children in Onondaga County and coordinates the delivery of follow-up services to children identified with elevated blood lead test results.
Typical Work Activities:
The Outreach Worker will assist program staff with the completion of necessary correspondence and data entry into electronic client and property records databases. The Outreach Worker will also conduct follow-up phone contact with clients, community agencies, and medical providers to promote care coordination and connect clients with beneficial resources in the community. The Outreach Worker will provide information about the LPPP via individual encounters and community outreach activities.
- Working knowledge of local agencies or programs that provide transportation, day care or medical services.
- Ability to prepare simple narrative and numerical reports.
- Ability to establish effective working relationships with health professionals, nurses and program participants.
- Ability to follow oral and written instructions.
- Ability to understand and interpret written material.
Full-time / 35 hours per week / 5 day work week
Occasional weekend and evening hours
